Deed Poll for adults

If you are not a citizen of the European Economic Area or the UK, you must get a Change of Name Licence before enrolling a Deed Poll. This licence is issued by Immigration Service Delivery.

  1. Complete the Deed Poll form

    Fill out the Deed Poll form in front of a witness who is at least 18 years old.

    Write your old name exactly as it appears on your birth certificate, including all middle names.

    If you have changed your name before, include this in your old name and attach proof — for example, a previous Deed Poll or your marriage certificate.

  2. Get your documents certified

    Make certified copies of:

    • your birth certificate
    • your photo ID
    • any supporting documents, such as your marriage certificate

    Uncertified copies will not be accepted.

    You can get documents certified by:

    • An Garda Síochána (police)
    • a practising solicitor
    • a Commissioner for Oaths
  3. Have the affidavit sworn

    Your witness must sign a sworn statement (affidavit) confirming they saw you sign the Deed Poll.

    This affidavit must be sworn in front of either:

    • a practising solicitor
    • a Commissioner for Oaths

  4. Pay the fee

    The Deed Poll must be stamped with €60 in stamp duty. Stamp duty is a government tax on legal documents.

  5. Enrol the Deed Poll

    Enrolling your Deed Poll adds it to the public Deed Poll Register. Enrolment is optional but may be required by some organisations.

    To enrol, complete the Request to Enrol Deed Poll form and include:

    • the original Deed Poll stamped with €60 stamp duty
    • a stamped, self-addressed return envelope

    Lodge your documents in one of the following ways:

    • In person: Book an appointment to lodge the documents at the Central Office of the High Court.
    • Virtual appointment: Book a virtual appointment before lodging. Email [email protected] for more information.
