Notice of Application for Personal Applicants

Tips for filling out the form

1. All sections (including Sections A - D) of the application form must be completed in all cases.
2. Include the full name of the person who died and any variations of the name on all documents.
3. Indicate the domicile of the person who died .
4. Include the names of all applicants in the personal application form.
5. Include only a photocopy of the will and any codicils.
6. Do not attach anything to the original will or codicils or remove any staples, pins, fasteners or binding when photocopying.
7. Include the original death certificate.
   
8. Ensure the Notice of Acknowledgement contains the details of all of the applicants that are listed in section C of the Personal Application Form.
9. Ensure all applicants have signed both the Notice of Acknowledgement and the Personal Application Form.
10. Include the last residential address of the person who died as well as the address in the will (if different).
11. Include the last residential address of the person who died, as well as the address in the will if different, at Section D of the application form, and include any other relevant former addresses of the deceased.
