Deed Poll for Minors (13 or Under)

If you are the parent of a child aged 13 or under and are a permanent resident in the Republic of Ireland, you can enrol a change of name by Deed Poll for your child. One parent completes the Deed Poll form and the second parent signs their consent to the change of name. Both parents must consent. If consent from either parent is not available, a court order may be required.

  1. Complete the Deed Poll form

One parent fills in the Deed Poll form. The second parent signs their consent. This must be done in front of a witness aged 18 or over.

Enter the child’s old name exactly as it appears on the birth certificate, including all middle names.

If the child has changed name before, include that change in the old name field and attach proof (for example, a previous Deed Poll).

Form: Change of Name by Deed Poll — 13 or under

  2. Get copies of documents certified

    Make photocopies of:

    • the child’s birth certificate
    • photo ID for the parent who completed the Deed Poll form
    • any supporting documents (for example, a parent’s death certificate)

Have these copies certified by one of the following:

    • An Garda Síochána
    • a practising solicitor
    • a Commissioner for Oaths

Uncertified copies will not be accepted.

  3. Complete the affidavits

The witness who saw the Deed Poll being signed must swear an Affidavit of Attesting Witness before a practising solicitor or a Commissioner for Oaths.

The parent providing photo ID for the application must also swear a Supplemental Affidavit before a practising solicitor or a Commissioner for Oaths.

Forms: Affidavit of Attesting Witness (13 or under) ; Supplemental Affidavit

  4. Pay the fees

These fees are paid as stamp duty (a government tax on legal documents):

    • Change of Name by Deed Poll document: €60
    • Supplemental Affidavit: €20

Find offices that provide stamp duty services

  5. Enrol the Deed Poll

Enrolling adds the Deed Poll to the public Deed Poll Register. Enrolment is not required by default, but some organisations may ask for an enrolled Deed Poll.

    To enrol, submit:

    • the original Deed Poll stamped with €60 stamp duty
    • the Supplemental Affidavit (stamped with €20)
    • a stamped, self-addressed return envelope

Form: Request to Enrol Deed Poll
