Deed Poll for minors aged 14-17

If you are aged between 14 and 17 and permanently live in Ireland, you can enrol a change of name by Deed Poll. You must complete the Deed Poll form and both parents must sign their consent. If consent from either parent is not available, a court order may be required.

  1. Complete the Deed Poll form

    Fill out the Deed Poll form in front of a witness who is at least 18 years old.

    Write your old name exactly as it appears on your birth certificate, including all middle names.

    If you have changed your name before, include this in your old name and attach proof — for example, a previous Deed Poll.

  2. Get copies of documents certified

    Make photocopies of:

    • your birth certificate
    • photo ID for one parent
    • any supporting documents (for example, a parent’s death certificate)

    Have these copies certified by one of the following:

    • An Garda Síochána
    • a practising solicitor
    • a Commissioner for Oaths

    Uncertified copies will not be accepted.

  3. Complete the affidavits

    The witness who saw the Deed Poll being signed must swear an Affidavit of Attesting Witness before a practising solicitor or a Commissioner for Oaths.

    The parent providing photo ID for the application must also swear a Supplemental Affidavit before a practising solicitor or a Commissioner for Oaths.

  4. Pay the fees

    These fees are paid as stamp duty (a government tax on legal documents):

    • Change of Name by Deed Poll document: €60
    • Supplemental Affidavit: €20

  5. Enrol the Deed Poll

    Enrolling adds the Deed Poll to the public Deed Poll Register. Enrolment is optional but may be required by some organisations.

    To enrol, submit:

    • the original Deed Poll stamped with €60 stamp duty
    • the Supplemental Affidavit (stamped with €20)
    • a stamped, self-addressed return envelope

    Lodge the documents

    • In person: Book an appointment to lodge the documents at the Central Office of the High Court.
    • Virtual appointment: Book a virtual appointment before lodging. Email [email protected] for more information.
    • By post: Central Office of the High Court, Four Courts, Inns Quay, Dublin 7. Note: Minor applications cannot be sent by post unless a solicitor’s cover letter is enclosed. Without this, the application will be returned.
